    New Old Stock Delta Manufacturing No. 943-B Back Plate For 4" Chuck To Fit No.950? or a No. 930 Lathe? Part of the number is missing.
    I am not real familiar with this equipment.
    Below Is what is written on the front of the box, as best I can read it. Note in the ( ) the word is unreadable but I believe it may be sized or tapered.
    The Diameter of the Pilot of This Chuck Adapter is Slightly Larger Than The Recess in the Chuck Back Adapter On Your Lathe Spindle and  (sized or tapered ?)    down to fit the Recess In Chuck.
    Do not Attempt To Force The Adapter Pilot Onto The Adapter Recess
    The Threads appear to be 3/4" in diameter and the plate is 1/2" thick and heavy.
